fischer might not have been totally dead after being shot he could have been in the state saito was in for most of the film also remember when cobb mentions in the film that he wanted to go deeper? he mentions to ariadne constantly that he didnt know how far he would be down there everything cobb knows then mal knows, projection or not she can ruin the whole idea its one of the biggest reasons cobb stated to ariadne that he couldnt build the dream at all
