I'm going to scan square one tomorrow and fix you too! But for now here are the notes that make up the chords for fix you:

 

Eb Major = Eb G Bb

Gm = G Bb D (In the book it has Eb in the chord though i.e G Bb D Eb)

Cm7 = C Eb G Bb

Bb Major = Bb D F

Ab Major = Ab C Eb

Eb/G = Eb G Bb (With G as left hand bass note)

Bbsus4 = Bb Eb F

Bbadd11 = Bb D F Bb F

Abadd9 = Ab C Eb Ab Bb

 

I think that's all of them and i hope that helps for now :)
