It's not just a case of depression, it sounds more like depression is a symptom of the real deal. I am just suggesting, I am not a pro, but this all sounds like a classic case of schizophrenia. And no, schizophrenia is not a multiple personality disorder. Quoting from wikipedia:

 

Schizophrenia is a mental disorder characterized by a disintegration of the process of thinking and of emotional responsiveness. It most commonly manifests as auditory hallucinations, paranoid or bizarre delusions, or disorganized speech and thinking, and it is accompanied by significant social or occupational dysfunction. People with schizophrenia are likely to have additional (comorbid) conditions, including major depression and anxiety disorders.

 

Antipsychotic medication might work better than anti-depression pills.
